Direct Primary Care

• We DON’T take any form of health insurance to pay provider fees 
• We DO love to see patients that have insurance. You need healthcare insurance for hospitalizations, ER and specialist visits, X-rays, Labs, Meds, etc., but you don’t need it to see us.
• If you don’t have insurance, we’d love to see you anyway. We’ve negotiated the lowest prices we could for meds, X-rays, labs and specialty care.
• We charge a one-time registration fee and low Monthly Fee – that covers most of the care we give.
  You don’t pay by the visit! Why do we do this?
  

1. We can focus on you and not the need to bill every possible penny at each visit. 
2. You never have to worry about having enough money to come to the office.
3. You are not penalized for being sick by having to pay more at the doctor and make less while missing work.
4. Because we know the monthly cash-flow and don’t pay staff to file insurance, we don’t have to see as many patients as possible each day to keep the office running.
5. With fewer patients needing to be seen, we can always get you in the clinic when you need to be seen. 
6. We’re also more willing to take care of your issues by phone of via secure messaging so you don’t miss work.

• We offer low-cost Lab Tests and Procedures – labs that your insurance pays $70 for, probably cost us (and now you) as little as $4.
• We do NOT prescribe routine narcotic (for pain) or benzodiazepine (for anxiety) medications for any NEW patients. If that’s what you need, this practice is not right for you.

One time registration fee of $50

Ages:

0-18 $35/mo

19-29 $45/mo

30-65 $80/mo

65+ $90/mo

Family membership $215  (includes 2 adults and 3 children under the age of 18)

Single Parent $150  (with 3 children) 

Additional children and out of town students are $10/month

Couples - 10% discount applied to membership

Membership includes 1 physical  with labs (cbc, cmp, lipids, TSH) per year in addition to any other wellness visits, chronic disease management, acute needs.  

Covered tests/procedures:

-Urinalysis

-Rapid strep

-Rapid flu

-Urine pregnancy (up to 2 per year)

-Urine test for protein for diabetics (up to 1/year)

-Rapid test for RSV

-Hemoglobin A1C (up to 4/year)

-Electrocardiogram (EKG)

-Pulmonary Function Testing (PFT)

-Removal of Cerumen (ear wax)

-Removal of benign skin lesions/warts*

-Joint injections (Knee, shoulder, hips)**

-PAP smear/Well woman care***

*Tests are preformed at the discretion of clinical staff

Patient will be responsible for charges by pathology lab if lesion is sent for analysis (can be billed through insurance if the patient has insurance)

**Patient will be responsible for charges for analysis of fluid and cost of medication injected into the joint

***Patient will be responsible for charges by pathology lab for analysis of specimen (can be billed through insurance).

For patients with insurance labs, prescriptions, imaging, and specialist visits are billed through insurance in the usual manner.  For those without insurance pricing for labs run in our office are negotiated and we have a reduced price list which will be available and posted to the website when available.

This membership DOES NOT include functional medicine care. DPC members are able to receive at 50% discount on functional medicine services but this is in addition to the monthly DPC rates.  See Functional Medicine page for more details on fees and services.
